Our Care Coordination services are provided by LUK’s Family Networks Lead Agency of the North Central Department of Children and Families (DCF) and by LUK’s Integrated Care Partnership (ICP). These services are available for specific populations and help individuals and families find services and resources that address a particular need.
LUK’s ICP services include Intensive Care Coordination and Family Support Partners through Massachusetts’ Child Behavioral Health Initiative. These services are subcontracted to LUK from Community Healthlink, and are available in North Central MA and the Worcester area.
