The Transition Case Manager works under the direction of the TIL Program. This position will successfully engage and retain young people in after care referred by LUK placement program(s). Through assessments and consultation, the Transition Case Manager will work with the young person’s team of internal and external entities, also known as collateral contacts, to ensure the young person transitions into the next level of care as seamlessly as possible. This position will build connections between LUK and key collateral partners to ensure the young people achieve their transition goals while adhering to LUK’s core values.
The Transition Case Manger will have familiarity with community resources in the region and will provide assessments and referrals as indicated.

Purpose
To work with young people to ensure that aftercare is an integral part of discharging planning from LUK placement care to ensure the young people continue to have stable living.
Responsibilities Include
- Acquiring a deep understanding and commitment to LUK’s Mission, Vision and Values and programming and advocacy
- Develop and maintain an authentic professional, trauma-informed relationship with young people
- Participates in the young person’s discharge plan from their placement program.
- Conducts a complete after care intake, including but not limited to information gathering and coordination with referral sources.
- Establishes working relationships with other providers to meet the specific needs of the young person.
- Develops, implements and/or monitors and reviews the young person’s individualized transition plan.
- Assists with transportation access and makes home visits as required.
- Assists in appropriate discharge process as formulated in the transition plan.
- Provides crisis intervention.
- Participates in on-call rotation.
- Collect and enter data as indicated
- Other duties as assigned
Knowledge and Experience Required
- Bachelor’s Degree or equivalent experience
- Demonstrated understanding of youth development, case management and service navigation required
- Experience with and understanding of older adolescents and young adults, families, and communities is essential
