The TIL Services Outreach Case Manager works under the direction of the TIL Services Program Coordinator. This position will successfully engage and retain young people between 16 and 25 in services. They will identify the strengths and needs of young people who are experiencing a housing crisis. The Outreach Case Manager connects young people to community resources to meet their needs. This position will gather information from young people via screenings, assessments, and consultation with collateral sources. With the young person, the Case Manager will develop goals for an individualized service plan. Evaluation of progress toward these goals as well as discharge planning and implementation are expected. This position will build connections between LUK and key partners and work with young people to achieve their goals while adhering to LUK’s core values.
A Bachelor’s Degree in Human Services or related field is preferred, but relevant experience may be substituted. Experience with housing/HUD, case management, and/or experience working with youth and young adults preferred. Strong relationship-building skills required. Excellent organization, communication, and computer skills required. Valid driver’s license and own transportation required.
Full-time positions located in Fitchburg and Worcester are available. Positions require community-based travel and implementation.

Purpose:
The TIL Services Outreach Case Manager identifies the strengths and needs of young people who are experiencing housing instability and helps them achieve their goals related to safety, permanency, well-being, and self-sufficiency.
Responsibilities Include:
- Provide case management and supportive services to youth and young adults who are experiencing a housing crisis
- Engage and retain young people in services
- Complete intakes and discharges in a timely manner
- Conduct assessments, service planning, case coordination, and make linkages with mainstream resources
- Enroll young people in services via LUK’s formal intake process utilizing our electronic health records system
- Meet with young people at a LUK office and/or community-based locations regularly
- Ensure young people are aware of expectations regarding safety and cleanliness of apartment unit through regular on-site visitation/ monitoring and skill building
- Maintain communication with young people/families between meetings
- Identify the needs of young people/ families and connect them to community resources to meet those needs (warm hand-off preferred)
- Develop working relationships with stakeholders to identify young people eligible for services and community resources to meet their needs
- Complete regular reviews of progress toward goals to quantify functioning
- Model skills for young people and coach/mentor them to develop their own skills
- Provide opportunities for young people and families to gain knowledge and skills in both individual and group settings
- Actively participate in supervision, trainings, meetings, and other project activities as required
- Participate in project evaluation activities and reporting
- Participate in emergency services on-call rotation
- Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent clinical services
Knowledge and Experience Required:
- A Bachelor’s Degree in Human Services or related field is preferred, but relevant experience may be substituted.
- Case management, and experience working with youth and young adults preferred
- Bilingual/Bicultural candidates and individuals with lived experience in housing instability and/or system involvement encouraged to apply.

Job Location:
