The CFSS Case Manager is central to the coordination and delivery of services to typically developing and neurodiverse youth/young adults/adults involved with the Department of Developmental Services (DDS), Department of Children & Families (DCF), or Department of Mental Health (DMH). This position works with the participant and their family (as indicated), and/or treatment team to identify their needs and connect them to community resources to address those needs. The Case Manager meets regularly with each individual and family (as indicated) to ensure progress toward goals on the treatment/action plan, to reduce barriers to success, and to evaluate progress at least every 90 days. The Case Manager works with State Agency Service Coordinators and Social Workers and the three domains of home, school/work (as indicated) and community to help develop and maintain a supportive web of care.
The qualified candidate must demonstrate an understanding of or readiness to learn trauma informed care. This position must demonstrate an understanding of impact of Adverse Childhood Experiences on family systems; substance use or addiction; mental health and trauma; cultural identity; gender identity; high risk behaviors and the impact of collateral trauma and marginalization. Strong relationship-building skills required. Excellent communication and computer skills required. Valid driver’s license, reliable and safe transportation required.
The qualified candidate will have a Bachelor’s or higher degree in Human Services, Counseling, Social Work or closely related field and at last one year of case management experience. Excellent communication and computer skills required. This is a full-time position located in Central Massachusetts and will include community-based travel and implementation. Purpose:
The CFSS Case Manager provides strength-based services to young people and their families to support progress toward their goals.
Responsibilities Include:
- Meet with young people and their families at their home, school, or other community-based location regularly
- Maintain communication with young people, their families, and other treatment team members between meetings
- Coordinate with the program coordinator to complete intakes and discharges in a timely manner, to include screening and assessments as well as individual treatment/action plan development
- Identify needs of the participants and their families and connect them to community resources to meet those needs (warm hand-off preferred)
- Complete regular reviews of progress toward goals to quantify participant functioning
- Model skills for young people and families and coach/mentor them to develop their own skills
- Actively participate in supervision and training as required
- Provide services that respect each family and youth’s cultural background, heritage, and identity
- Participate in emergency services rotation
- Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent services
Knowledge and Experience Required:
- Bachelor’s Degree in Human Services, Counseling, Social Work, Psychology or closely related field
- 1 years of experience providing case management

Job Location:
