Our Care Coordination services are provided by LUK’s Family Networks Lead Agency of the North Central Department of Children and Families (DCF) and by LUK’s Community Services Team (CST). These services are available for specific populations and help individuals and families find services and resources that address a particular need.
LUK’s CST services include Intensive Care Coordination, Family-based Intensive treatment, and Family Support Partners through Massachusetts’ Child Behavioral Health Initiative. These services are available in the North Central and Worcester regions of the state through Together for Families (TFF). Together for Families is a Community Services Agency (CSA) Partnership between LUK, Inc. and BHN.
