Together For Families

Together for Families (TFF) is a Community Services Agency (CSA) Partnership between LUK, Inc. and BHN. This program serves the communities of Central Massachusetts, including the North Central and Worcester regions.

TFF services are for families with youth under age 21 who are experiencing significant behavioral health challenges in the following areas: Home, School & Community. This can include:

  • – Needing an IEP, 504 Plan, or other accommodations;
  • – Having serious issues with relationships;
  • – Being unable to function in public settings;
  • – And other serious impacts on life.

We can help you determine if your family meets this eligibility, and help get a qualifying diagnosis as appropriate.

There are three main TFF Services:

Family Partner

A Family Support and Training Partner (Family Partner) is a parent or caregiver who is raising, or has raised, a youth with special needs. Through their own life experience, they have learned about the human services system and know the challenges that come with raising children with specific special needs.

Family Partners provide a supportive “parent-to-parent” relationship. They can mentor parents as they attempt to better understand their child’s needs and obtain help and services for their family.

Many of our Family Partners also have experience working with schools, DCF, housing authorities, the juvenile court system, and other community-based organizations.

ICC (Intensive Care Coordination)

Intensive Care Coordination (ICC) is a service that uses Wraparound care planning to coordinate multiple services and supports for youth. Involvement and participation in ICC is voluntary. A care coordinator is assigned to work with each youth and family, and families can also be connected with a Family Partner if desired. The care coordinator is responsible for working with the youth and family to:

  • – Make a risk management and safety plan.
  • – Conduct a home-based assessment of the youth’s and family’s strengths and needs.
  • – Assemble a care planning team to help support the youth and family. This includes both formal (e.g. teachers, state agency workers, etc.) and natural (e.g. friends, neighbors, etc.) supports of the family’s choosing.
  • – Organize and create an individual care plan (ICP) that guides the family and care planning team in pursuing the goals they identified
  • – Make sure services and supports on the ICP are in place and coordinated.

FIT (Family-based Intensive Treatment)

Family-based Intensive Treatment (FIT) is a service that includes intensive family therapy, youth-focused case management, and Family Partner support. The main focus of FIT is to improve the youth’s mental health issues, strengthen the family structures, and increase supports.

FIT services aim to safely transition youth into less intense, community-based treatment so families can feel stable sooner. They also aim to prevent the need for more intensive treatment, like hospitalization or inpatient psychiatric treatment.

FIT services, including family therapy, are delivered in the home and community instead of an office setting. Families also have access to 24/7 On-Call support for youth stabilization.

Eligibility

Together for Families is a program for youth enrolled in MassHealth Standard and/or MassHealth CommonHealth insurance plans. We can help determine if you are eligible.

Together for Families serves Central Massachusetts, including:

North Central: Ashby, Ayer, Barre, Berlin, Bolton, Clinton, Fitchburg, Groton, Hardwick, Harvard, Lancaster, Leominster, Lunenburg, New Braintree, Oakham, Pepperell, Princeton, Rutland, Shirley, Sterling, Townsend

Worcester: Auburn, Boylston, Grafton, Holden, Leicester, Millbury, Paxton, Shrewsbury, Spencer, West Boylston, Worcester
