Youth Support & Tracking

Youth Support & Tracking (YST) is a service provided to children between the ages of 5 and 22 who are involved with the Department of Children and Families. It offers a range of intensity from a low level (1 ½ to 2 ½ hours of service per week) to a high level of intensity (7 days per week of contact), direct and indirect. Services are typically 13 weeks.

YST services include:

  • Referral & Intake
  • Youth Support and Tracking
  • Linkage to Community Engagement
  • Case Management
  • Service Planning/Transition Planning
  • Crisis Management/24 Hour On-call
  • Modeling for parents
  • Discretionary Funds to Offset Outside Activity Costs
  • Assessments
