Per Diem Case Manager (Fitchburg area) - LUK provides Individual Support Services to those over the age of 18 living with Autism or an Intellectual Disability. The ideal Case Manager is consistent and organized, and willing to make a commitment to the involved participant. Reliable transportation is also a requirement of the position as travel is expected. Case Managers are responsible for outreach to a variety of community agencies and providers, and for maintaining/updating documentation online as required. Afternoon/evening/weekend availability is preferred. Experience with DDS population and fluent in ASL encouraged to apply.
