LUK Inc.'s Intensive Foster Care Program (IFC) will provide intensive foster care placement and support services for Departement of Children and Families children and youth (ages 0-22). IFC will provide a network of skilled foster homes that will meet the DCF credentialing guidelines, coupled with a service team, consisting of a Case Manager and Family Resource Specialist, which is designed to meet the individual needs of the child/youth, and his/her family. This program will have the ability to provide placement services to children/youth whose placement needs are sufficiently met within a traditional foster care setting. This program will provide the referred youth, the family and the treatment team with the opportunity to individualize services to best meet the youth's/family's needs. IFC will provide foster homes that can meet these unique needs, from homes that specialize in short-term placements both in times of emergency or as transitional placements, to homes that can provide long-term placement options for youth as well as homes that understand the special needs of youth aging out of the DCF system. These foster homes can also function in the roles of a mentor or sponsor. In addition, youth that are determined to need additional supports to remain in the DCF foster home will be provided with support services by the Case Manager through the unbundled case management component of the contract.
